		<description>I would suggest weaning from the bottle and the formula at the same time.  I would not put the milk in the bottle, I would put it in a straw cup. At the same time I would start watering down the formula until it does not taste as good as the whole milk.

Eliminate one bottle at a time. And instead offer the cup. For some babies this is easy and the bottles can be eliminated quickly and for some the process is slow. Each day after 12 months it becomes harder and harder.  

I did this with my daughter starting at eleven months.  For her it was an easy transition.</description>
